Athletic Forms needed to participate in athletics:
Parent Consent form (parent fills out and returns to Athletic Director or Coach)
Interscholastic Sports Medical Form (doctor fills out and you return to Athletic Director or Coach)
NYCDOE Consent to COVID-19 testing - (scroll down until you see consent. This can be done directly the website or you can print out a hard copy)
*Please note if you are planning to participate in Girls Volleyball or Coed Bowling you will be required to upload proof of Full Vaccination.*
